REMINDERS!!!
Incoming Freshman are now required by the State of Tennessee to go to their primary care provider for a Well-child Exam so they can not get their physical forms completed at these events. You can find the new ruling at http://health.state.tn.us/sportsphysical/index.htm
The Preparticipation Medical Evaluation Form (Sports Physical forms) must have the medical history on the first page completed and SIGNED by a Parent/Guardian prior the physical. Please bring these completed histories and signed forms with you to your physical.
Who can attend:
Incoming 10th, 11th, and 12th graders– Fall and Winter Sports
We will not be giving sports physicals to athletes younger than 10th Graders.
IMPORTANT CHANGES IN PHYSICAL REQUIREMENTS FOR INCOMING FRESHMAN ATHLETES!
The Tennessee State Board of Education has passed a new ruling stating that all incoming Freshmen athletes will need to complete an Early, Periodic Screening, Diagnosis, and Treatment (EPSDT) or well-child exam. This will include 7 components listed below:
- Health History
- Behavior and Mental Health Assessment
- Complete Physical Exam
- Lab tests that are individual specific (i.e. urinalysis, blood work, etc)
- Vision and Hearing Screening
- Immunizations
- Anticipatory Guidance- Tobacco, Drugs, and Seat Belts
These student athletes will need to make arrangements to get the EPSDT done through their primary care physician or pediatrician. The clearance form and more information are available at http://health.state.tn.us/sportsphysical
It is important to note that these athletes will also need to get their doctor to fill out and sign a TSSAA Physical Form in order to be eligible. It is recommended that incoming Freshmen take their TSSAA forms with them to the primary care physician and ask that both sets of forms be completed when they go for their EPSDT screen.
